When it comes to Toronto Bridge with bar we mean a variant of the classic prosthesis screwed onto implants: the implants are connected by a milled titanium bar on which the prosthesis is fixed. How the bar variant works
In the classic Toronto Bridge the metal structure is integrated inside the prosthesis and everything is screwed directly to the implants. In the variant with bar a step is added: the laboratory mills one from solid titanium bar which is screwed onto the implants and rigidly connects them to each other; the prosthesis is then fixed on the bar (screwed or with precision attachments, depending on the project).
The result is one cross connection of the implants: the chewing forces are distributed over the entire bar instead of on the single implant, and the prosthesis has an extremely stable and passive support.
When it is indicated (and when not)
The bar is especially useful when implants are positioned with different inclinations (typical of protocols with inclined posterior implants): the bar compensates for divergences and guarantees a perfect passive coupling. It is also indicated with long arches, with high masticatory forces or bruxism, and when you want the maximum possible rigidity of the system.
It is not always necessary: in many arches the classic Toronto Bridge on 4-6 implants, with integrated titanium structure, already offers excellent stability at a lower cost. The bar affects the price because it requires CAD/CAM milling from solid and more laboratory hours: having an internal dental laboratory, this surcharge remains contained in our price list, while with external laboratories (as often happens in Italy) the item weighs more on the final estimate. Advantages and limitations, in summary
Advantages: load distribution on all implants, passive coupling even with inclined implants, maximum rigidity, possibility in some projects to make the prosthesis more easily removable by the dentist for maintenance.
Limits: higher laboratory cost, greater vertical space to manage in the project (space is needed for the bar + prosthesis), home hygiene which requires more attention under the bar (brushes and water flosser become daily allies).
