Why 6 systems instead of 4
The Toronto Bridge on 4 lifts (protocol All on 4) uses inclined posterior implants to bypass areas with little bone. The version on 6 plants it distributes the load over multiple support points: each implant works less, the prosthesis is more stable and there is redundancy — if one implant has problems over the years, the prosthesis remains supported by the other five.
Six implants are particularly suitable when: the bone is of good quality and volume (there is room to position them all at a good angle), the patient has a very strong chewing or bruxism, that is the arch to be rehabilitated superior with medium-good quality bone, or you want the maximum possible durability of the product.

4 or 6 systems: how you decide
The decision is clinical, not commercial, and is based on three elements evaluated with the 3D CT scan: bone volume and density available in strategic areas, length of the prosthesis (the more the prosthesis extends towards the bottom of the arch, the more posterior support is needed), e masticatory forces of the patient.
With poor bone in the posterior areas, 4 inclined implants are often the best choice because they avoid grafts. With good bone, 6 implants give a higher margin of safety. Those who always and only propose 6 implants - or always and only 4 - probably start from the price list and not from the CT scan.
Procedure and times
The operation with 6 implants lasts approximately 2-3 hours per arch under local anesthesia (sedation available on request). The protocol is the same as the 4 implant version: extractions if necessary, insertion of the implants, fixed temporary within 24-72 hours when primary stability allows it, definitive after 3-6 months of osseointegration.
